Booking An
Appointment
Our service area includes London and Middlesex, Oxford
and Elgin Counties.
If you live in London
Please come to the office at 125 Woodward Avenue*
(see map under "Locations"), to complete a brief application form.
We will give you a list of things to bring to the appointment, and
schedule a private, confidential assessment for you with one of
our counsellors. The cost is $10.
If you live outside the city
You may download the application
and expense form and e-mail
them to us or fax it to us at 519-433-4559. Once you have done so,
please call our receptionist at 519-433-0159 x 7151
to schedule the appointment. We will mail out an appointment confirmation
to you, along with a list of things to bring to the appointment.
