Community Integration Program
This program works to provide planning and support services to developmentally challenged individuals
and their families. Services are based on respect, informed choice and will be inclusive of natural supports.
They will offer informed choice, be non intrusive and value individual strengths. All services are voluntary.
Population Served
These services are offered to individuals 18 years of age and over and their families who have a developmental
disability. They are available to individuals and families who reside in London, Middlesex and Elgin County and
are funded by the Ministry of Community, Family and Children's Services.
Services Available
- Resource Information and Referral - Comprehensive information and referral to appropriate
community service for individuals with a developmental disability.
- Case Management –- Staff will assist individuals and families to develop support plans
of their choosing.
- Advocacy –- Staff will advocate with or on behalf of people having difficulty accessing
service.
- Crisis –- Staff are available to become involved in situations where there has been
a drastic change in support needs.
- Community Development/Education –- Staff are available to assist individuals and families
to link with natural community supports.
- Life/Future Planning –- Staff will facilitate planning towards a written plan that
can be used for the pursuit of resources.
- Criminal Justice Coordination –- Case management services for individuals with a developmental
disability involved with Criminal Justice System.
